Vendor Information Form

Thank you for participating in the CSCC 2018 Annual Conference. Sponsors and Exhibitors are listed on the conference app that is available to all conference attendees, and we invite you to provide as much of the following information about your organization that you would like to have posted on the app.

Note that the contact name and coordinates should be the person that you would want the conference delegates to follow-up with if they have a question about your products and services. The description field can contain information about your company, and/or about specific items that you will be displaying at the conference – the content in this field can be whatever you want to include.
